- Bachelor’s degree in business or related field
- Previous experience in marketing, preferably in home health care
- Excellent verbal, written communication skills
- Demonstrated organizational skills, flexibility, assertiveness, and team orientation
- Demonstrate creativity in fostering new referral relationships to increase client census
- Prior experience as a Community Liaison in Home Health highly preferred
- Organized, adaptable, and confidently proactive
- Collaborative team player with innovative outreach strategies
- Work full-time Monday through Friday, occasional evening and weekends for marketing events
- Work collaboratively with clinical staff to assure consistent message to the customers and the community
- Build and maintain relationships with referral sources, payers, and the community
- Represent the agency at events, health fairs, and networking opportunities
- Track and follow up on referrals for smooth transitions and communication
- Educate the community on home care and aging-in-place solutions
- Guide prospective clients and families through the care process
- Obtain required discharge info to complete client integration processes
- $50,000 to 80,000, dependent on referrals and performance
- Referral bonus structure
