As an LPN with our team, you’ll play a key role in providing quality home healthcare, allowing patients to remain independent at home in the healthiest and happiest state of being possible.

What you’ll be doing:

  • Provide physician-prescribed nursing care and treatments.

  • Administer medications, perform dressing changes/wound care, and monitor vital signs.

  • Observe, record, and report patient conditions and reactions to drugs or treatments.

  • Participate with all other home care personnel in patient care planning.

  • Return the individual to an optimum and productive level within the patient’s capability.

What we’re looking for:

  • A passion to serve and help others live their best lives possible.

  • Location: Ability to provide care to patients in Fort Collins, CO.

  • Education: Graduate of an accredited Practical Nurse program.

  • License: Currently licensed as an LPN in the state of practice.

  • Experience: One (1) year of experience preferred in a home health setting.

Schedule:

  • Day shift

  • Monday to Friday: 8:00 am – 5:00 pm

  • Weekends as needed

License/Certification:

  • LPN License (Required)

  • CPR Certification (Preferred)

Compensation: $31.00 - $35.00 per hour
